Motivation for change

Motivation  for change is an effective way of helping you to plan how you want to move forward with your pain self-management.  

It’s gentle approach helps you determine:

    1. what it is you want to do
    2. plan the next steps in your self-management 
    3. review your successes

We offer two approaches self-directed and assisted.

Self-directed is for the people who can make changes to their lifestyle.  Typically you make decisions about supports and services that help you to: Enjoy meaningful relationships with family and friends, Experience health and growth, Live in the home and community you choose, and Work, volunteer or do activities you enjoy.

Assisted is for people who would like a little help in making changes.
